Below are the various forms that are required to participate in the Navy JROTC program at Dunedin High School.

  • NJROTC instructors are required to take a copy of this form with them anytime a cadet goes off-campus for a unit event or activity. This allows medical treatment by emergency personnel in the event of injury or illness.

    Standard Release Form

  • The military holds itself to the highest standards—often exceeding those expected in civilian settings—and we strive to uphold those standards with openness and transparency. This commitment extends to our Navy JROTC instructors and cadets.

    Educating the next generation is a responsibility we take very seriously. This document clearly outlines the expectations and responsibilities for both JROTC instructors and cadets, ensuring a safe, professional, and supportive environment for learning and growth. As parents ourselves, we take pride in holding our program to the highest standards among educators.

  • All cadets are required to complete a SPORTS physical, rather than a standard school physical, in order to participate in the Navy JROTC program.

    Sports Physical Submission

  • This document is required by the Department of Naval Education & Training to ensure cadets are physically fit to participate in NJROTC activities. These activities include the Cadet PT Challenge, field trips, summer camps, and all team events.

    If a cadet answers “YES” to any question on the form, it must be reviewed and signed by a licensed medical professional before the cadet can participate in program activities.

    Health Questionnaire

  • Proof of school insurance: Required if your cadet is on any teams or plans to attend any field trips or activities.

    Please be sure to save the receipt to your phone/computer.

  • Florida Law requires that all cadets participating in the Navy JROTC program at Dunedin High School are required to complete three online courses every year.

    Please turn the certificates of completion in via Canvas, email or hardcopy. If already completed for another sports/activity you may print out that same certificate and turn it in.
